Mlawa, August 21, 1917

 Before the below undersigned official of the civil registry for religious matters of non-Christian belief appeared today the tradesman Henoch [Chanoch son of Nutuh] Nutkiewicz (Nutkevitz), age 38, residing in Mlawa (at) ul. Plocka 6, of Jewish belief and notified in the presence of the witnesses: the synagogue sexton Berek Katowic, age 62 residing in Mlawa and the tradesman Chil Reingewirc (Raimi), age 23 residing in Mlawa, that to his wife Roza [Ruchle daughter of Yitzhak Yakov Ha-Levy] Nutkiewicz (Nutkevitz) née Tachna residing in Mlawa, of Jewish belief, in Mlawa, in the apartment of the announcer on the eleventh of August, nineteen hundred and seventeen, in the morning at eleven o’clock was born a child of the male gender and received the name Izak Jakób [Yitzhak Yakov son of Chanoch].

 Before the below undersigned civil registrar for members of a non-Christian creed appeared today the tradesman Henoch [Chanoch son of Nutuh] Nutkiewicz (Nutkevitz), 38 years old, residing in Mlawa, Plocker str. 6, of Jewish creed and notified in the presence of the witnesses the synagogue sexton Berek Katowic, 62 years old residing in Mlawa (and) the tradesman Chil Reingewirz (Raimi), 23 years old, residing in Mlawa, that to Roza [Ruchle daughter of Yitzhak Yakov Ha-Levy] Nutkiewicz (Nutkevitz) née Tachna, his wife, residing in Mlawa, of Jewish creed, in Mlawa, in the apartment of the announcer on the eleventh of August, nineteen hundred and seventeen, in the morning at eleven o’clock was born a boy and that the child had received the first names Isaak Jakob [Yitzhak Yakov son of Chanoch].
