English translation:
It happened in the town of Pultusk on the twenty eighth of
February/twelfth of March, eighteen hundred and eighty three at three
o'clock in the afternoon. Nuta Nutkiewicz, trader, residing in the
town of Pultusk, twenty seven years old, appeared in the presence of the
witnesses, the synagogue sextons, Abram Cinamon, sixty two years old,
and Josek Lejb Karas, fifty two years old, both of whom residing in
Pultusk, and showed us a child of male gender, declaring that it was
born in Pultusk at the third / fifteenth of May, eighteen hundred and
eighty at five o'clock in the afternoon to his wife Dina née
Finkielsztejn, twenty four years old. At circumcision this child
was given the name Gienuch (Henuch). This document was read to the
testifier and the witnesses and signed by them and us. The delayed
registration of this document was due to the absence of the father.-
